FAQS 
What do I need to bring?  Please bring the prescription given to you by the physician, your current insurance cards, an x-ray or MRI report of current condition or injury if available, a current medication list and any co-pay if this applies.  Please arrive at least 15 minutes prior to your appointment to complete the required paperwork if not printed from our website under "patient forms and brochures."
If you wish to print our forms and bring them with you, please print the following:
  • Patient Registration Form
  • Patient Medical History
  • HIPAA & Medical Release Form
  • Financial Form
  • Patient Injury/Illness Intake Survey

If your injury is due to a motor vehicle accident, please bring the car insurance information to ensure proper billing. If your injury is due to an accident at work, please have the case manager or adjustor contact our office to provide the needed information.
 
What should I wear?  Patients are advised to wear comfortable loose clothing and comfortable shoes. Depending upon your condition or injury, please wear something which makes the area accessible for the therapist to provide needed treatment.
 
What will my first visit be like? Your first appointment will begin with an evaluation. You will receive an overview of your condition or injury, along with needed modifications to your activities.
 
How long will my visit(s) last? Generally, an appointment will last 45 minutes to 1 hour, depending upon your condition or injury. The first appointment for an evaluation may require you to be here longer.
 
How often do I come? Typically, a patient will come 2 to 3 times per week, depending upon a patient's schedule and the physician order. We will make every effort to accommodate your schedule.
 
Do I have visit limits with my insurance? Certain insurance companies regulate the total number of visits. We advise you check with your insurance company.
 
Will I have a co-pay? Physical therapy treatment may be subject to a deductible, co-pay, or co-insurance. These will vary depending upon your specific insurance policy.
 
How do I pay my co-pay or make payments to my account? We accept cash, checks, Visa, Mastercard, Discover, and American Express.
 
How long will my therapy take to complete? Your physical therapy will be coordinated with your physician and will provide an assessment of your condition, frequency of treatment, short term and long term goals for your therapy and any precautions which are needed. Based upon communication between the physician, physical therapist and you, the length of therapy will be decided. Please remember physical therapy may require several visits to return to you to your normal activities and lifestyle.
